Aged Care
Stay ready for the Commission every week of the year
The strengthened Quality Standards raised the evidentiary bar, and they apply continuously, whichever week the assessors arrive. Gwenda reads your operational systems and shows your standing against each standard as it moves.
The strengthened Aged Care Quality Standards
Built around the strengthened Aged Care Quality Standards, in force since 1 November 2025. Gwenda reads rostering, care management, training and incident systems to show your standing against each of the seven standards.
Discovery
What Gwenda finds, and where it finds it
Every finding names the system it came from. That is what makes it evidence rather than an assertion.
Worker screening and training that is about to lapse
Police certificates, statutory declarations, professional registration and mandatory training tracked per worker, per service, to the expiry date. Read from HR and learning management systems.
Rostered hours against care minutes
Registered nurse coverage and direct care minutes delivered against the targets your service is required to meet. Read from Rostering and workforce management.
Incidents against SIRS timeframes
Reportable incidents identified and checked against notification windows, with the ones approaching a deadline surfaced first. Read from Incident management, care management systems.
Clinical indicators trending the wrong way
Falls, pressure injuries, significant weight loss, restrictive practices and medication errors, read from the care record with nothing re-keyed. Read from Care management systems.
Complaints that have not closed the loop
Feedback and complaints without a resolution, an owner, or evidence that practice changed as a result. Read from Complaints register, care management.
Care plans and consent records
Assessments and care plans past review, and consent or supported decision-making records that are missing or out of date. Read from Care management systems.
The benchmark
The Gwenda gold standard for aged care
Conformance with all seven strengthened Quality Standards evidenced continuously, workforce screening and training current across the roster, and clinical governance able to demonstrate outcomes rather than intentions. A provider needs to hold that position every day, including the long stretch between site audits.
- The person at the centre
- Evidence that care is delivered with dignity and respect, that older people direct their own care, and that consent and supported decision-making are recorded.
- Governance and accountability
- A governing body that receives and acts on quality and safety information, with the reporting to prove it, and accountable owners for each standard.
- Workforce currency
- Police checks, statutory declarations, registration, mandatory training and competency current for every worker on the roster, with expiries surfaced ahead of time.
- Clinical care and high-impact risks
- Falls, pressure injuries, restrictive practices, medication management and the Quality Indicator Program measures tracked with the evidence behind each.
- Incidents and the SIRS obligation
- Incidents recorded, assessed and, where reportable, notified inside the Serious Incident Response Scheme timeframes, with the analysis that follows.
- Feedback and complaints
- Complaints captured, resolved and demonstrably used to change practice, closing the loop the Standards expect.
- Food, environment and daily living
- Evidence against the standards covering nutrition, the service environment and the residential community.

Outcomes
What changes for you
A standing position, held all year round
Conformance against each of the seven standards, updated as your operational systems change, with the evidence attached.
Reportable incidents caught inside the window
SIRS timeframes tracked from the moment an incident is recorded, so a notification deadline is not discovered after it has passed.
Board reporting that writes itself
The quality and safety information a governing body is expected to receive, assembled from the source systems with the trend across periods.
Workforce compliance without a spreadsheet
Screening and training currency across every worker and every site, surfaced before someone is rostered without a current clearance.

The strengthened Quality Standards changed what evidence looks like in aged care. The existence of a policy used to be enough; providers are now asked to demonstrate outcomes for the person, and the Standards apply every day of the year, site audit or none.
That is a continuous evidentiary burden, and it falls on systems that were never designed to produce evidence: rostering, care management, incident registers, learning platforms. Gwenda reads across them and holds the position for you.
Questions we get asked
Do you cover the strengthened Standards or the old ones?
The strengthened Quality Standards, which took effect on 1 November 2025 under the Aged Care Act 2024. There are seven of them and they replaced the 2019 Standards. Gwenda is built against the current set.
Does this cover residential care, home care, or both?
Both. Several standards apply across all service types, and Standard 7 applies specifically to residential services. Gwenda scopes each standard to the service types you actually deliver.
Does Gwenda replace our care management system?
No. Your care management system stays the clinical record. Gwenda reads from it to evidence your position against the Standards, and reads alongside it from rostering, HR and incident systems.
How does this handle multiple services?
Each service holds its own position against each standard, with a provider-level view across all of them. A problem at one site is not diluted by good performance at another.
